Our mission
True To You Outreach helps vulnerable individuals stabilize during difficult life transitions through supportive guidance, navigation, and connection to essential resources. Our work prevents crises, homelessness, from escalating into emergency system involvement or recidivism and supports individuals as they move toward stability, accountability, and safer, more secure futures.
How our mission makes a real difference
We focus on real outcomes, not just services.
Participants complete structured programming and receive a certificate of completion, demonstrating accountability and progress. We support individuals in securing employment, connecting to housing, and accessing behavioral health and substance use treatment when needed.
Through consistent guidance, resource connection, and follow-through, individuals leave with a plan, support system, and the tools needed to move forward and reduce the risk of returning to the system.
Empowering the Community
We show up by listening first, without judgment or assumptions. We help individuals and families understand what support is available, navigate next steps, and stay connected to resources that reduce stress and uncertainty. When appropriate, we assist with coordination, planning, and problem-solving before challenges escalate.
We provide steady, consistent support focused on stability, follow-through, and prevention. This includes helping reduce crisis, prevent homelessness, limit disruptions, and support forward movement for both individuals and the families impacted alongside them.
We also support youth and families during early stages of concern. When parents feel overwhelmed, lose hope, or struggle to manage behaviors influenced by negative environments, peer pressure, mental health challenges, or substance use, we step in with guidance, structure, and support. Our goal is to help redirect that path early and strengthen both the individual and the family.
Because sometimes all it takes is one person who believes in you and the people who stand beside you.
Our story
True To You Outreach was created from real-life experience, not theory.
Someone very close to me was incarcerated for several years, and when they returned home, I saw how difficult it was to transition without a plan, support, or clear direction. There were real barriers, real setbacks, and moments that could have led them back into the system.
It took grit to move forward.
That experience revealed a gap. Too many people are expected to figure everything out on their own during one of the most critical transitions of their lives.
True To You Outreach was built to fill that gap by providing early, structured support through our recidivism prevention and reentry initiative.
Because no one should have to walk out and figure it out alone.
